1915(i) Community Living and Supports

Breaking Barriers Behavioral Services, LLC offers Community Living and Supports through the NC Medicaid 1915(i) program to help individuals develop the skills necessary to live as independently as possible within their homes and communities.

These services focus on enhancing independence, improving quality of life, and increasing participation in meaningful community activities.

Community Living and Supports may include:

  • Daily living and self-care skill development

  • Personal hygiene and grooming support

  • Meal planning and preparation

  • Household management skills

  • Money management and budgeting assistance

  • Communication and social skill development

  • Community integration and participation

  • Transportation training and community navigation

  • Employment readiness and vocational skill support

  • Development of healthy relationships and self-advocacy skills

Services are individualized based on each person's strengths, goals, and needs and are delivered using a person-centered approach that promotes dignity, independence, and community inclusion.
